Below is a sample of the form we need for insurance.  Please only provide this form, and not a copy of your policy as we cannot accept that.

This is called an Accord Certificate of Liability Insurance.  It must show your club name, dates of coverage, at least one million dollar ($1,000,000) per each occurrence. It also must have "Owosso Tractor Repair & Parts" and "Mackinac Bridge Authority" as certificate holders in the bottom left corner.
